Accident report
Students must file an accident report if they sustain injuries requiring medical assistance while performing a task at a University institution, or while on the way to or from that institution (e.g., commuting directly between home and the University).
Read the explanation carefully before filling out an accident report. After you have completed the Accident Report, submit this to the academic office.
Note that you need to use your STiNE username to log into the Staff Service Portal.
